Where we come from
Western Avenue Pharmacy is a forward-thinking specialty pharmacy in Glendale, California, operating 24 hours a day across acute care, post-discharge, and long-term care. For most of our history, we've worked deep inside the higher-acuity end of healthcare — skilled nursing facilities, hospice care, hospital discharge follow-up. We've watched what happens when independence breaks down. We've also watched, again and again, how often it didn't have to.
What we kept seeing
The same story repeated itself: a senior arrived in the emergency room after a fall, a confusion episode, a medication mishap. Almost always, when we traced it back, something small had set it in motion — a side effect dismissed as aging, a new prescription causing dizziness, two prescribers unknowingly duplicating therapy, a dose taken at the wrong time.
Most of these were not catastrophic medical events. They were preventable medication issues that no one had caught in time. And by the time we met those patients, the damage was already done.
Why we built The Prime Initiative
The Prime Initiative is our answer. It's a community-facing program designed to reach people before the crisis — while they're still living independently, still managing their own medications, still navigating their own healthcare.
We go directly into senior living communities, retirement complexes, and independent living facilities. We bring breakfast. We bring our pharmacists. We bring real, practical information that residents can use the same week. There's no cost, no sales pitch, and no pressure.
The philosophy
The name comes from a simple idea: if there is one thing pharmacy should be working hardest to protect, it's a person's independence. Their right to manage their own life, their own home, their own decisions, for as long as they're able. That's the prime directive of our practice.
Every program we build, every facility we visit, every resident we meet is in service of that one goal — extending the prime years of life for as long as possible.
